Information Request Form

This form will allow you to submit requests for the following:

  • Building/Parking Lot Use Inquiries
  • Building Tour Inquiries
  • Press Release Requests
  • Operational Inquiries (operational/administrative consulting, our processes & procedures, organizational structure, etc)
  • Ministry / Program inquiries (suggestions for ministries/programs you would like to see @ Roots)
  • Broadcast/Production Inquiries (set-up, software, hardware, CDNs, equipment lists, equipment recommendations, etc.)
  • Creative Inquiries (branding, brand coaching, brand guide, creative team structure, creative philosophy, training curriculums for photography, video editing, graphic design, etc.)
  • Speaking/Worship Engagement Requests (rootsWORSHIP, Pastor Jason Gaszak, Roots Church Teaching Pastors, rootsCREATIVE, etc.)
